TO: All North Allegheny Student-Athletes and Parents/Guardians FROM : Robert Bozzuto – Director of Athletics SUBJECT: 2011 – 2012 Physical Procedure To be eligible to play a sport during the 2011 – 2012 school year, the initial CIPPE physical evaluation must be completed no earlier than June 1, 2011. Any CIPPE physical turned in after that date will be valid for the entire 2011 – 2012 school year. This comprehensive evaluation can be performed at your physician’s office. When making an appointment with your family doctor, please take all physical forms to the authorized medical examiner and have him/her fill out Section 4 of the evaluation. For an initial sport physical, please fill out and return Sections 1, 2, 3, 4 and 8 to the Athletic Office located in Senior High School. Please make sure that your physician has signed and dated the bottom of Section 4 of the CIPPE form. Also, please be sure that your physician has listed the blood pressure and pulse rate. If the student-athlete’s yearly physical was performed before June 1, 2011, there are a number of walk-in clinics that will administer a school physical. The first is North Park Urgent care at 9401 McKnight Road – Pittsburgh 15237, 412-366-1528; Med Express Urgent Care at 6998 Crider Road – Mars, PA 16046 (off of Rt. 228) – 724-778-3627. Another clinic is the Walgreens also located on Rt. 228 in Mars right near the Med Express. We are also offering a clinic with our school doctor. Dr. Robert Potter’s office will be conducting physicals for North Allegheny student-athletes ($30 each) on Saturday, June 25th from 8:00am to 12:00pm at Northern Area Family Medicine – 5700 Corporate Drive – Suite 700, Pittsburgh 15237 – 412-630-2670. All forms must be sent directly to the Athletic Office, not handed to the coach, no later than August 8, 2011. Physicals may be turned in anytime throughout the summer and school year. Physicals turned in at the last minute may result in the student not being permitted to practice or to participate in tryouts. Such delays may result in a loss of tryout time or perhaps losing the opportunity to tryout completely. When a student-athlete has a desire to participate in a sport that has roster reductions or cuts, it is imperative that they have all of their paperwork submitted in a timely fashion. Each coach will receive an updated alphabetical list of all students who turned in a valid physical to the Athletic Department. For any athlete participating in multiple sports within a school year, a re-certification form needs to be turned in for the 2nd and/or 3rd sport played (winter and/or spring sports’ season). For the 2nd and/or 3rd season, the parent/guardian and student will need to re-certify their contact information and health assessment by filling out Section 5 of the CIPPE physical form. Our Certified Trainers will set up clinics whereby all wrestlers can have their weight certified. Please refer to the North Allegheny website’s athletic page for those dates. There is no need to go back to your physician for a recertification UNLESS the student-athlete has sustained a serious injury since the initial physical. If this is the case, then both Section 5 and Section 6 must be turned into the Athletic Office a week prior to the start of the winter or spring sports’ season. Lastly, the North Allegheny Athletic Department mandates that all students participating in athletics complete the UPMC Impact Concussion Baseline Testing Program prior to the first day of official practice. This innovative process known as IMPACT, which stands for Immediate Post Concussion Assessment and Cognitive Testing, is mandatory for every North Allegheny student-athlete entering grades 7, 9, and 11 for the 2011 – 2012 school year. Students entering 8th, 10th and 12th grade do not need to take an ImPACT test for the 2011 – 2012 school year unless they did not take the test during the 2010 – 2011 year (including the summer 2010 ImPACT testing dates). The schedule for all fall 2011 sports will be released on our website at www.northallegheny.org/athletics. The schedules for winter and spring sports will be available closer to the start of each season. All ImPACT testing sessions are administered in the High School Computer Labs located on the 2nd floor of the Senior High School. There are two weeks of testing offered at no charge for each sport’s season. Additional make-up testing may be offered at a cost of $25 per student.
